- Form SS-4 (Application for Employer Identification Number): This form is used to apply for a tax identification number for your business. An identification number is required for any business which files income tax forms separate from its owner(s).
- Form W-4 (Employee's Withholding Allowance Certificate): This form must be completed by each employee to determine the amount of federal income tax that should be withheld from the employee's pay. Completed forms must be kept on file by the employer.
Louisiana Department of Revenue
- Application for a Louisiana Tax Number: This online application is used to apply for a Louisiana tax identification number for your business. An identification number is required for any business which files sales/use tax or withholding taxes.
- Form L-4 (Employee Withholding Exemption Certificate): This form must be completed by each employee to determine the amount of Louisiana state income tax that should be withheld from the employee's pay. Completed forms must be kept on file by the employer.

I-9 (Employment Eligibility Verification): "All U.S. employers
are responsible for completion and retention of Form I-9 for each individual
they hire for employment in the United States. This includes citizens
and noncitizens. On the form, the employer must verify the employment
eligibility and identity documents presented by the employee and record
the document information on the Form I-9."
